Dr. L. Lee, Director of the Centre for Family Medicine Memory Clinic provided a presentation to the Committee via teleconference concerning the Primary Care Collaborative Memory Clinics in Ontario.

 

Dr. Lee discussed slides concerning the following topics:

 

                     The history of the development of Primary Care Collaborative Memory Clinics;

                     A chart illustrating problems in the health care system for those suffering with dementia;

                     The results of studies concluding that dementia destabilizes other complex chronic conditions;

                     A map illustrating the Primary Care Collaborative Memory Clinics established throughout Ontario;

                     The services available in a typical primary care practice;

                     The services available in a Primary Care Collaborative Memory Clinics;

                     The benefits and structure of a Primary Care Collaborative Memory Clinic;

                     The statistics across Ontario associated with Primary Care Collaborative Memory Clinics;

                     Information on the Centre for Family Medicine - Memory Clinic Training Program;

                     A chart detailing the sustainable and efficient care for dementia patients;

                     The essential elements to provide a gold standard for health care; and

                     Testimonials provided by patients / families associated with care received at the Primary Care Collaborative Memory Clinics. 

 

In closing, Dr. Lee discussed the need for Primary Care Collaborative Memory Clinics in the City of Barrie and surrounding areas.

 

Members of the Committee asked several questions related to the information provided and received responses from Dr. Lee.
