Dr. L. Lee, Director of the Centre for Family Medicine Memory Clinic provided a presentation to the Committee via teleconference concerning the Primary Care Collaborative Memory Clinics in Ontario.
Dr. Lee discussed slides concerning the following topics:
· The history of the development of Primary Care Collaborative Memory Clinics;
· A chart illustrating problems in the health care system for those suffering with dementia;
· The results of studies concluding that dementia destabilizes other complex chronic conditions;
· A map illustrating the Primary Care Collaborative Memory Clinics established throughout Ontario;
· The services available in a typical primary care practice;
· The services available in a Primary Care Collaborative Memory Clinics;
· The benefits and structure of a Primary Care Collaborative Memory Clinic;
· The statistics across Ontario associated with Primary Care Collaborative Memory Clinics;
· Information on the Centre for Family Medicine - Memory Clinic Training Program;

Mr. D. Piper and Mr. B. Logan provided a presentation regarding Connecting Seniors with Young Adults in the Community Program.
Mr. Piper and Mr. Logan discussed slides concerning the following topics:
· The background on a Non-Profit Organization named “Beyond Potential”;
· The organization’s goal to “Add Value to People’s Lives” through leadership, volunteering and building relationships between seniors and young adults;
· The inspirations for creating the program; including:
· Personal experiences;
· Understaffed seniors residences,
· Loneliness and health issues of seniors;
· Young adults from high school in need of volunteer hours; and
· Young adults to establish value through volunteering.
· The short term and long term goals for the volunteer program in our community; and
· The development of a software management program for young adults in need of volunteer hours for high school.
